Planning for eligible students should mirror the regular transition process. In Tennessee, planning for transition begins at age We refer to this period as the preparation phase — getting the student ready to benefit from services. Consultation should intensify as the eligible student moves further along the school path. Services for eligible students could be added to the transition page of the IEP no later than 16 years of age.
VR counselors should work with appropriate school personnel to determine how they plan to disseminate VR information to students and their families.
Transition means: 1 the formal and physical process of transferring from school to school or 2 the period of time in which a student moves from one school in the sending state to another school in the receiving state. Individualized education program means the written plan of instructional goals and objectives developed in conference with an individual less than 21 years of age, their parent or legal representative as applicable , teacher, and a representative of the public school district.
Development Plans means a coherent set of operations defined and financed exclusively by the OCTs in the framework of their own policies and strategies of development, and those agreed upon between an OCT and the Member State to which it is linked;. Dependent care assistance program or "DCAP" means a benefit plan whereby school employees may pay for certain employment related dependent care with pretax dollars as provided in the salary reduction plan under chapter Transition Services Agreement has the meaning set forth in Section 3.
Project Implementation Plan means the detail plan submitted by the Developer with regard to development of Project Facilities and its operation and management thereof in accordance with this Agreement and to be appended as Schedule 9 to this Agreement. Program services means services that include all of the following provided they are pursuant to a program agreement: program needs assessment and development, job task analysis, curriculum development and revision, instruction, instructional materials and supplies, computer software and upgrades, instructional support, administrative and student services, related school to career training programs, skill or career interest assessment services and testing and contracted services.
Prescription monitoring program means the electronic system within the Department of Health Professions that monitors the dispensing of certain controlled substances. Via works with students, in partnership with their school districts and the Office of Vocational Rehabilitation, to create a personalized employment plan and explore careers of interest, preparing them for the next step after high school — community, integrated employment.
Students explore careers of interest with expert, one-on-one guidance from Via staff. Our experienced team develops sites within local business, industry and non-profit settings, offering students work-based learning experiences or job trials. Students gain valuable work skills and develop insights about adulthood throughout this process. To be eligible for this service, students must be between 14 to 21 years old, and must be enrolled in high school in Lehigh, Northampton, Berks, Carbon, Monroe or Pike county.
This work experience helps the students to develop the work and social skills that are necessary prior to their placement at a job site in the local community. The students gain school-based work experience in the following areas:. Students are given the opportunity to work at a local participating business that has agreed to partner with us.
